Hey folks, make sure you completely fill out the form! We need all fields completed! If you have problems putting your name on the first line, but it with your screen name on the second line. Thanks a bunch folks.

I believe it sends a courtesy copy to yourselves as well once the form is signed. Just to be safe, print that and bring it with you.
