Claying and Polishing are done as needed (same with wax, although many people will use a schedule so they replenish their wax prior to it wearing out).

With claying, inspect your paint feel on occassion. When it no longer feels smooth (you feel pumps, grit, and/or a pebble-like texture) it is time to clay.

With polishing, the best indicator is that you can see visible swirl marks in the paint. Polish to remove these.
