In gear (1st or reverse) with the parking brake on... This depends on facing uphill or downhill...

parking brake always - it's just a good habit to have

Indeed they are. Went to New York once in January when I had my Mazda3 and put the parking brake on. Got down to 0 F that night and the brake was frozen.

facing uphill first and parking brake, downhill reverse and parking brake.

Other than that its first and parking brake...

I do it the way I was taught when I went through school for driving Manual...

1st gear on uphill + ebrake..
R on downhill + ebrake...
neutral on flat + ebrake...

if there is even a slightttttt angle creating even the slightest uphill or downhill angle... I follow the above and no longer consider it... "flat"...

8-24 Driving and Operating

Parking
If the vehicle has a manual
transmission, before getting out of
the vehicle, move the shift lever into
R (Reverse), and firmly apply the
parking brake. Once the shift lever
has been placed into R (Reverse)
with the clutch pedal pressed in,
turn the ignition key to LOCK/OFF,
remove the key and release the
clutch.
If parking on a hill, or if the vehicle
is pulling a trailer, see Driving
Characteristics and Towing Tips
on page 8-49.

Uphill - 1st, downhill - reverse, flat - 1st or reverse depending on which direction I'm headed first. Oh and always, always, always with ebrake engaged.

When rolling down to the metro Detroit area, I just grab a couple dead sewer rats and chock a tire. The long tails make great handles for slapping those critters into a firmly wedged position. Steeper grades I use muskrats. And then tie off a pissed off live one to a wheel spoke for an urban security system.

1st gear, parking brake on. I had a problem with the parking brake latch on the goat last year (a common issue on GTOs) and until I fixed it I had to do 1st gear only. Drove me crazy.
