Are we single or three bolt factory?
I ordered a BTR stage 4 cam it came and it has 3 bolts in a line next to each other. One on each side and a bigger one in the middle. I thought ls3's were single bolt? Is that right? They didn't mention I would have to swap to anything on the phone. Said it was the same as my factory setup? Am I looking at this wrong or no

You are correct. The factory ls3 is single bolt. You will need a three bolt cam gear.

I'm pretty sure you are looking at the back of the cam. The front of the cam has a locating pin that stick out of the face. Factory is a single bolt as said before. A three bolt will have them arranged in a triangle pattern around the center of the cam. This is a three bolt cam.
