The only way I can imagine this happening is if the brass locking keeper was installed incorrectly or not installed at all. I almost put it in wrong on mine, I didn't realize you have to push it in pretty hard untill it clicks and snaps into place. Either that or maybe the whole locking mechanism wasn't pushed all the way inside the cap? Do you have a picture of what it looks like after it came out?

Don't have pic but if it happens again I will. I followed the directions to the letter and its keyed correctly as the key works great! I'll try to push it in further and turn it to ensure it's seated properly.

Does anyone know if this same install technique works on the lock cylinder for the glove box? Ours was missing and we ordered a new one. It "appears" to be the same setup. Just not 100% sure.

I think it does. I believe it uses the same cut positions as well.

make sure you have a spring under the brass keeper as well.

someone reported having a bad cap in the last couple of months, I would make sure there is a groove inside the cap that will capture the brass keeper.

I have the tumblers all correct and the lock works. When I insert the brass piece it doesn't. The brass piece does not click into place. Then when I put the key in the lock will not work. It seems that the brass is preventing the key from going all the way down. Any ideas?

Nevermind.
I just found out that it helps if you put the brass piece in the CORRECT direction.

Thanks for a great post on how to do this...

I received my locking gas cap and used your post for instructions.. Made it very easy and pictures were totally helpful.

Thanks, man... had it done in ten minutes, with most of the time spent cutting open the G.D. bags

I bought mine from Maureen at Rogers Chevrolet, arrived quickly. Thank you Maureen.
I got mine all ready to go, it took about 25 minutes, the hardest part was the brass keeper, I had to use some force to push it in, other than that the next step is to install the clip onto the fuel door.
