What is the best shifter and 3.91 gear to buy?

I upgraded to the MGW shifter and am very happy with it, can't help with the gears.

MGW, Barton, or the factory Hurst. I honestly don't think there's been a bad review of the MGW. The others have differing opinions.
AAM (American Axle & Machine) for the gears.

MGW hands down. You'll miss 1st to 2nd before you a miss a 2nd to 3rd in a MGW.
