hitting rev limit

When i start out and punch the gas, it will pull to 3500rpm, then it seems as if something gets stuck, and revs to the limiter if i let it. If i pull a little off the gas pedal it shifts, then quickly shifts to the next gear.

This has been going on a while, and i have tried two tunes, but they do not correct it. I am mostly sure it will be hardware. I have a throttle body and halltech cai. I do not have to Punch it, i just need to take of spirited for it to act up.

Has anyone else had this issue or seen it? Can a throttle body cause this issue by sticking open?
