My initial thought is just that a bulb blew out on the driver side and they dropped in a replacement that doesn't match, and might not have actually put the bulb in completely....

You are safe just mixing 50-50 and running it until it's full. I doubt you deal with -10° weather where a 60/40 water/coolant mix would be. Boiling points are negligible between these ratios.
