I agree with the other responses. Sounds like a head gasket let go. Maybe not a huge hole was opened up though-- hence the relatively small coolant loss. Hard to get around the white smoke. Basically, only a head gasket failure of some type will cause that.
As far as the shaking only when in the 5-6 gears-- just a guess, but maybe since your power is way down now, with at least one clylinder dead, it would make sense that the engine is really straining to move at higher speeds in the overdrive gears. While in the lower gears the trans still allows enough of a mechanical advantage to crutch the power loss in the engine. Just my thoughts.
Is it making any loud rattling or clanging sounds with the engine rpms?
