Mindz, you can use CutePDF Writer to save the changes:

On the left of the above link, you will see two links: Free Download and Free Converter. Get both but make sure to "run" the converter first, then run the install for CutePDF Writer. It will act as a printer, so when you are done filling in the form, go to File --> Print --> and select CutePDF Writer. A "Save" dialog box will show up. Save it and done!
