post up a close up of the fracture surface and I may be able to tell you how it failed.
does it look like this:

my guess its that it does look similar (fatigue cracks), and the vibration was to much for the moment arm. A better grade bolt would work... but a relocation would be best as you will still be fighting that moment arm.
