They were applied with 3M tape and not sure how it may have come off. It was just the one in the middle of that picture. I am guessing the tape did not stick too well or something. All the other are fine. The ones I want to replace them with will be a lot cheaper and are vinyl and will look the same. Reason I didn't go with the vinyl ones originally was cause I ordered those first and found out about the vinyl ones after.
