I installed the IPS kit which is similar to what you got. I "T'd" the nipple from the drivers side(1 in your pic) to the nipple on the LS2/3 valley cover and hooked them to the center nipple on the catch can. The nipple on the intake by the TB(2 in your pic) I have hooked to the outer nipple of the catch can. The other outer nipple on the catch can I have hooked to the air filter of the turbo. I just drilled a small hole on the end of the filter itself as it goes directly over the compressor wheel cover inlet and attached the line there.
The nipple at the front of the passenger side valve cover I have capped off and I put the optional breather on the oil fill cap. You will not need the nipple that is on the charge pipe(? in your pic) any more.
That is how SC2150 asked me to install it. I got the normal 3 nipple catch can but will be replacing it with the Monster catch can soon.
