Just got a stone chip on mine yesterday, taking it to the glass place to see if they can fix it before it cracks.

Road hazard. I try and not draft or trail other vehicles. Especially trucks. My niece just had her 2002 Camaro hatch glass shatter while parked. I suspect her lift support died of old age and became a rocket.

Depends on the state. Some states allow for free replacement of the glass through insurance. some don't. In SC, it is free.

Though I tend to allow for plenty of distance between myself and other vehicles so it doesn't happen very often.

Twice for me already.

This happens to cars driven on roads.

In MN, liability covers glass with a maximum of $100 deductible. That being said - I always opt for $0 glass deductible.
