New E-Brake having an issue after a week of use

Hello all, hoping someone can chime in here. 6 months ago my original e-brake cable snapped, lasted me since 2010. A week and half ago I installed a new one. Worked great for about half a week but then it wouldn't click. Opened the side panel and noticed a piece was hanging inside. I've attached a picture of it and circled in red. I've put it back into place at least 5x now and it will pop back out of its "track" after a handful of e-brake pulls. Kind of running out of ideas here. I put some more grease on it thinking that would help keep in place but that hasn't worked. Any ideas on what to do next? This is not an aftermarket piece it's OEM if that matters.
