To answer your question, no its not common. Pistol failure is sometimes a bad tune or lack of fuel and the piston ring heats and expands in to the ring groove and the pistol breaks. Not a common failure on a stock LS3
Or if youre out of oil or low oil pressure of course, that would be a cause as well

It was not my car at the time it happened , but from what I was told it was running fine, shut it off started it and it happened. After I wrote this I was in the engine section on this forum and seen an engine build and it sounded like the same thing happened to him, right on start up. I’m not sure if an injector gets partially plugged, or is there anything to the steam ports not on all for corners and a cylinder getting hot and detonating ? Thx in advance

Over rev and the valves could hit the piston.

The valve springs go soft eventually from heat and use.

Piston or rod? The powdered metal rods aren't exactly bulletproof but still a pretty rare break in a stock motor.

This. What's the limit for like a factory LS3? I probably wouldn't even bother trying to ask about the limits of a bone stock L99....

What top end or bottom end parts tend to fail first? And apart from a bad tune, could oil pump/pressure issues cause certain parts to "grenade" in such a fashion?

The limit depends on a multitude of factors.

Tune, fuel quality, boost pressure if you go FI.

If everything is perfect

Heads and cam 550HP

Supercharger/Turbo 575 or so(6/8 pounds of boost)

Nitrous 150 shot

All pulled apart one piston completely broken off the rod and broken up in the pan, with the rod bent and valve way up in the head. It appears it dropped valve seat and everything went south from there. So could have been a lot worse, so hurt the head pretty good, some marks in the cylinder. So I will have to sleeve one cylinder, and a rod and a piston need replacing, one bent pushrods. I will change out all the lifters, oil pump, gaskets, timing chain and tensioner. Cam looks good.
