None that I could tell. I'd have to cut it open to really look at the plunger / walls of the valve bore as I couldn't move it.

I'd contact Melling and send it back. First time I've heard of a Melling doing this though. Interesting.

There are much less# of Melling pumps in use than OEM. Therefore, the number of failures should also be much less, even if the fail rate is same (which I have no idea about).

I thought the factory replacements were actually Melling pumps too? Very interesting about this happening with Mellings.

True but I have not heard of a Melling failing at all. Doesn't mean they don't of course.

On my last build I spent quite a bit of time porting and polishing my Melling 10296 and made a point of checking the relief piston to bore fit. I found no reason to be concerned. Wondering if this isn't a product of trash making it's way through the engine and seizing it up?

Factory pumps are made by Melling, but Melling brand have upgraded qualities over EOM. See post #52 in this thread.
