I'm gonna grab a noid light tomorrow and see how the signal looks coming to the injector. If the signal is jacked up it's either the wiring or the PCM.

Also check for spark from the coil pack. I know you swapped them out at one point, but it might be the ground to that coil or something like that.

Ok, so got the Noid light and plugged it in and I see a constant blinking which tells me it's getting signal from the PCM. I haven't swapped out the coil pack yet though, so that's next on my list.

Is it possible the PCM is sending signal now but in other circumstances it isn't?

Update! So I think it's wiring to the injector. Earlier today I had the noid light hooked up and it blinked, but very dim. I just tried it again after driving the car around for a bit and this time the light was blinking super bright. I thought that was odd since the first time it was very dim, so I tugged on the main black cable that all the driver-side injectors go into and the noid light cut off entirely. I wiggled it some more and the light came back on full strength. So, I'm gonna have to cut into the thing holding the cables and see what I can find.

What was your final fox for this? I know this was nearly 6 years ago but I’m having the same issue.
